Eligibility Criteria

• Individuals age 64 1/2 or younger on the assigned effective date.
• Any child must be 3 months of age or older to be eligible for a child-only plan if not a dependent with an adult subscriber.
• You must be a resident of the state of Florida.

Dependent Eligibility Criteria

• Your lawful spouse
• Dependent children are covered up to the end of the calendar year in which they reach age 25 provided the child is dependent upon the applicant for support AND is living in the Applicant’s household OR is a full-time or part-time student. Dependent children who have reached the end of the calendar year in which they turn age 25 can continue to be covered up to the end of the calendar year in which they reach age 30 provided the child is unmarried and does not have a dependent of their own AND is a resident of Florida OR a full-time or part-time student AND is not covered under any other health insurance policy or entitled to Medicare or Medicaid.
• Your own or your spouse's unmarried children, regardless of age,enrolled prior to 19, who are incapable of self support due to continuing mental retardation or physical disability and are chiefly dependent upon you for support and maintenance. Proof of disability and dependency is required.

Plan Type Overview

• Open Access Plan (New!)- flexible in and out-of-network coverage under a national network. No PCP or referral required.
• Health Savings Plan (New!) - high deductible plans allow you to use a tax-advantaged Health Savings Account (HSA) to help pay for your current medical expenses or save for future medical expenses. In and out-of-network coverage under a national network. No PCP or referral required.
